COVID-19 Safety Questionnaire

Have you traveled out of the country in the last 14 days?



Have you been in contact with with anyone who has traveled out of the country in the last 14 days?



Have you knowingly come in to contact with anyone who has tested positive for COVID-19 in the last 14 days?



Do you currently have a cough, fever, runny nose or flu like symptoms?



Does anyone in your household currently have a cough, fever, runny nose or flu like symptoms?



The following are the new rules and regulations that apply to services with F.A.B. Services, I will need you to carefully read and agree to each point. These form part of your agreement with me and your service is contingent upon them:

I will greet you at the door and open it for you. Please allow me to put Max in his Kennel first.



I will have and sanitizer ready for you. Please do not touch anything (like walls or doorknobs) until you have used the hand sanitizer. This will help your appointment to start on time as all surfaces that are touched will need to be cleaned before your service can start (remember I have small kids to worry about)



Please place your belongings on the floor in my nail room and head to the washroom to wash your hands. I will have a new clean towel on the chair for you to take with you to the washroom. Please bring it back with you when you are done and do not use the towel that is in there. (This is a family towel and while I am sure that it will not have come in to contact with the virus it is for your protection).



If you have a mask, please be wearing it when you get to the door. If you do not have a mask, one will be provided to you free of charge as long as supplies last.



I ask that cell phones stay in your pocket or your purse while you are here. If you have unknowingly come in to contact with the virus, that is the most likely place for the virus to linger.



Until further notice, the ability to book your appointments online has been suspended. I am not accepting new clients until a vaccine has been found for COVID-19.



At this time I will not be accepting cash as a payment method. I now have the ability to accept debit cards, along with Credit Cards and E-Transfer.



Due to the cost of the increased cleaning procedures and the fees associated with accepting debit, I have had a slight price increase. (I have not had a price increase since September of 2013. The increase ranges from $1 - $5.00 depending on the service and how often it is performed).



And, Finally it is imperative (with all the extra cleaning procedures I have in place) that you are on time for your appointment. I have extended the appointment times to accommodate the additional cleaning , but if you are not on time, it may delay everyone after you. I have amended my late and no show policy to reflect 8 minutes grace. I will also now be unable to accommodate early arrivals as I have no where for you to wait safely. Please text me when you arrive and I will let you know when it is safe to come up. Please plan your time and travel accordingly.
